&lt;p&gt;&amp;nbsp;&lt;/p&gt;
&lt;p&gt;You can get the &lt;u&gt;forms&lt;/u&gt; from any number of places, including local courthouses, post offices, and certain other state/federal government offices.&amp;nbsp; If you&amp;#39;re in the military, or work for them you can also do the paperwork there.&amp;nbsp; I don&amp;#39;t know which office on base though.&amp;nbsp; Possibly whatever they call &amp;#39;personnel&amp;#39;.&lt;/p&gt;

If you are 16 and older it is $100 and is good for 10 years. If under 16 it is $60 and is good for 5 years. In my opinion, it is not worth the expense to get the cards as they are not good for anything other than land crossings. They are not valid for air travel so they will do you no good if you need to be airlifted from a port of call ! We got all of ours from the post office. There is a list on the travel.gov website of post offices that provide passport services. &lt;p&gt;This is the official website for US passports.&amp;nbsp; You can fill the form out online and then print it out.&amp;nbsp; The cost is $100 for an adult.&amp;nbsp; You can mail it in or go to a post office that has a passport agent and some smaller courthouses also do it.&amp;nbsp; You will need a photo ID, a certified copy of your birth certificate and 2 color photos.&lt;/p&gt;
&lt;p&gt;Just a hint on the photos, don&amp;#39;t wear light a light colored shirt when you have them taken, they tend to be to light and they will be rejected.&lt;/p&gt;
&lt;p&gt;When I applied for a new one in 2005, my original had expired in 1977, it took 2 weeks.&amp;nbsp; My husband was renewing his and it took 4 weeks.&amp;nbsp; I have heard from co-workers that it can now take up to 8 weeks so the sooner you apply the better.&lt;/p&gt;
